Degloving Approach for Total Excision of the Primary Malignant Melanoma of the Nasal Cavity

ABSTRACT

The primary malignant melanoma of the nasal cavity and paranasal sinuses is extremely rare in Korea. The majority of this malignancies are in either the anterior part of the nasal septum or the middle and inferior turbinate. The exact site of origin of many of the cases from the upper respiratory tract is difficult to determine, but the assumption is that the neoplasm originate from the melanocytes in the mucosa of the upper respiratory tract. The degloving approach, which consist of lifting the soft tissue from the mid portion of the face, thereby furnishing unlimited exposure to the pyrifrom fossae and lateral nasal wall, offers an excellent alternative to the lateral rhinotomy technique. The degloving procedure involves no external scarring and improves the visibility of the total operative field. The authors have experienced a case of primary malignant melanoma originated on the right nasal septum and extended to the lateral nasal wall, and the patient had treated by wide excision of the tumor with degloving approach and postoperative irradiation with good result.
