Statistical Observation to Middle Ear surgery of Chronic Otitis Media

ABSTRACT

A operative evaluation was done on 235 patients treated for otitis media at Dept. of Otolaryngology, Chosun University Hospital during past 3 years (from August 1980 to July 1983). The results were obtained as follows ; 1) Sex distribution was male 1.3 : female 1 and age distribution showed 95 cases (40.4%) in 2nd decade, 85 cases (36.4%) in 3rd decade, 28 cases (11.9%) in 4th decade. 2) No significant difference in site incidence (right or left ear) was noticed. 3) Duration of diseases was 99 cases (42.1%) from 6 years to 10 years, 53 cases (22.6%) from 11 to 15 year. 4) X-ray findings of temporal bone were 169 cases (72%) in sclerotic, 35 cases (14.8%) in cholesteatoma, 19 cases (8%) in diploic, 12 cases (5.2%) in pneumatic. 5) No significant relationship between hearing loss and duration of diseases was noted. 6) Large perforation was observed in 80 cases (34%), Moderate perforation in 67 cases (28.5%), small perforation in 53 cases (22.6%) and attic perforation in 17 cases (7.2%). 7) Hearing loss according to the tympanic membrane perforation was 40.27 dB in case of small perforation, 44.40 dB in case of doderate perforation and 50.14 dB in case of large perforation. Consequently, hearing loss showed increased trend with the size of perforation. 8) Routes of approach were 105 cases (44.8%) in postaural with Endomeatal incision, 80 cases (34%) in postaural incision, 33 cases (14%) in Endaural incision, 17 cases (7.2%) in Endomeatal incision. 9) Types of operation for middle ear surgery were tympanoplasty with mastoidectomy in 99 cases (42.1%), radical mastoidectomy in 89 cases (42.1%) and tympanoplasty in 29 cases (12.3%). 10) Conditions of ossicles were 163 cases (69.4%) in destruction and 72 cases (30.6%) in intact.
